Managers and Coaches please note:
All teams are required to be insured for coverage of accidents and liability. You should have received an application from the S.A. Van Dyk Insurance Company (1-800-323-7326). Van Dyk is the only authorized and approved carrier. A team cannot play if it does not have the Van Dyk policies. Please forward copies of the insurance (one for accidents and one for liability) with your Forms #1 and #2.
All National forms #1 & #2 must be typed then transmitted directly to your District Director no later then June 13th. Don't run the risk of having your team disqualified by not getting these forms to your District Director on time.
If you have a computer, you can go to The American Legion National Headquarters web site (www.baseball.legion.org). Then click on the drop-down menu called Rules & Forms. There will then be a link to the New ALB Forms. Find these forms, fill in the blanks, and print the forms out on your computer. They can not be submitted electronically.
Any incomplete forms sent to Department Headquarters will be immediately sent back to the District Directors and in turn they will mail them back to you.
